In healthcare, one size does not fit all. When providers have time to build relationships and dig into patients' health issues, it sets them up for better outcomes. As Dr. Beaver explains here, our providers spend more time with each patient. We use this extra one-on-one #PrimaryCare time to listen to and learn more so we can proactively treat holistic health needs instead of just symptoms. #PatientRelationship

Personal health goals help inspire a healthier lifestyle, but sometimes these goals stall when trying to put them into action. Community events like a recent dance class at one of our centers – discussed by Dr. Rene Roberts, MD, MS, FAAFP on the news – build accountability and partnership, making goals easier to reach. "It's so important that you feel free to tell your doctor everything." In her story, Portia emphasizes what we know: Great #PrimaryCare starts with doctors who are invested in their patients and see them as partners in their health journey. As a retired nurse and member of the LGBTQIA+ community, Portia appreciates having a care team she can trust and truly open up to. #PrideMonth

May is #MentalHealthAwareness Month, and a recent report shows mental health diagnoses have risen on a national level since 2019 — with the biggest rise coming from adults 65 and over. Overall health outcomes depend on primary care providers valuing #BehavioralHealth in tandem with physical wellness. At Oak Street Health, our integrated care model means supporting all aspects of our patients' health and well-being. That's why our patients are screened for behavioral health at their initial visit and annually, and our centers are staffed with behavioral health specialists, medical social workers, and psychiatrists to treat behavioral health conditions (Source: FAIR Health).
