We were just onsite in South Carolina with the Chronic Care Management team at Columbia Nephrology Associates. Our experts are proud to support the great care this team provides to kidney patients between visits. In just two months, the practice stood up a Connected Care program to enroll 1600+ patients with just 2 FTEs - putting them in the top 10% of all chronic care management programs in the country. Nephrology patients need significant care and support between office visits. Most practices do this work inefficiently and without compensation - but Columbia Nephrology has been able to rapidly scale their care management program across hundreds of patients while generating fair compensation. In fact, the practice received reimbursement for their April claims by mid-May, and their program success will fund another FTE.
