Santa Clara Family Health Plan is proud to share the selection of Sandra Walle into the National Academy for State Health Policy (NASHP) – Emerging Leaders of Color Fellowship program. Sandra’s journey with SCFHP began as a Quality Improvement Coordinator, to Community-Based Services Specialist, and now at the same time as a NASHP fellowship. We are honored to see Sandra’s dedication to improving healthcare access and social determinants in health for millions of Californians. We are excited to support her as she embarks on this opportunity.

Dear community, We opened a SCFHP Gilroy Service Hub. This hub provides convenient access to our Gilroy, Morgan Hill, and San Martin communities. "We’re excited to open this service hub in Gilroy to begin serving our members and residents in South County,” said Cha See, PhD, MPH See, SCFHP Director of Community Engagement. “This is a direct outcome of a community participatory process where we heard from residents and system partners for more service opportunities in South County. Santa Clara Family Health Plan has listened to member’s feedback and opened our Santa Clara Family Health Plan Service Hub in Gilroy. Located in the Neon Exchange Building, the Service Hub is taking appointments for Medi-Cal application and renewals and CalFresh application. This Service Hub supports our Gilroy, Morgan Hill, and San Martin communities. Please schedule an appointment before visiting by calling 408-613-9001. Location: The Neon Exchange at 7365 Monterey Highway. Gilroy 95020 Date and time: Mondays from 10 a.m. to 4 p.m. and Wednesdays from 10 a.m. to 3 p.m. #healthforall
