Today marks the start of National Health Center Week! This week recognizes the incredible benefits that Community Health Centers (CHCs) provide to the communities they serve. To kick off this week of celebration, let’s talk about the work that CHCs do with social determinants of health! Social determinants of health are the nonmedical factors that influence health outcomes. They are the conditions in which people are born, grow, work, live, and age, and the wider set of systems shaping the conditions of daily life. Understanding social determinants enables health care providers to transform care with integrated services to meet the needs of their patients and demonstrate the value they bring to communities.

Social determinants of health (SDOH) are the conditions in which we live that impact our well-being. These include where we live, work, learn, and play, as well as our access to resources like food, housing, education and healthcare. Addressing SDOH is crucial for improving health outcomes and reducing health disparities. By focusing on these underlying factors, we can create healthier communities and improve the quality of life for all.
