    #Heart_Failure has a massive impact on our society and the health care system. We as a system are unable to keep up with the increasing number of patients and the morbidity associated with this disease❗ 🔔 What would happen when #Heart_Failure care is "outsourced" to help with medication titration, follow up and prevention of HF hospitalizations? ❗ We now have such data In follow up: There were decreases in systolic blood pressure -7 and ⬆ in HF meds Did the additionally delivered care cost more money? No, 52% ⬇ in total cost of care (-$1,076.64 per patient/month) despite increase in $ due to PCP visits.     In a newly published manuscript in the Journal of Cardiac Failure, comprehensive remote patient monitoring and care improved clinical outcomes and reduced total cost of care in heart failure patients. Dr. David I. Feldman, cardiovascular medicine fellow at Massachusetts General Hospital and first author on the manuscript, shares of the results: "Our usual way of caring for heart failure patients isn't working. We need to consider all options when it comes to maximizing GDMT and helping patients with heart failure live longer, healthier lives. Collaboration with a centralized RPM program to optimize GDMT titration efforts is a necessary and impactful solution to help close heart failure care gaps and improve outcomes at scale."     If you’re curious about the ins and outs of the American Medical Association’s CPT Editorial Panel’s upcoming review of the 15 codes used for remote monitoring of patients, Medical Economics gives an in-depth explanation around the potential for new billing codes that could give physicians more flexibility — and reimbursement — for RPM programs. Proposed code changes could lead to payment for more of the time physicians spend caring for their patients. In his congressional testimony last month, Cadence’s founder and CEO Chris Altchek shared that our experience with Medicare beneficiaries and PCPs “has made clear that RPM is key to the future of primary care.” Cadence’s customer base is 95% primary care physicians who want to improve how they manage patients’ chronic conditions outside of office visits. The industry agrees that more flexible payment for these visits is important for patient access. Christopher J Adamec, vice president of the Alliance for Connected Care, a coalition of telehealth and RPM organizations added: “[Reimbursement for] a window of 11 to 20 minutes may not sound like much, but the time adds up—so much that the 20-minute threshold for reimbursement results in approximately 30% of care being uncompensated.” Read the full article here for more information on what this means and the decisions coming down the pipe: https://lnkd.in/dvWVzhW8

    Did you know that Cadence patients are, on average, 250% more likely to trigger an alert when they transmit a vital reading between 11 p.m. and 3 a.m? Cadence’s Medical Director, Randall Curnow, MD, MBA, FACP, FACHE, FACPE, explains, "patients often are most vulnerable emotionally and physiologically in the late evening and early morning, times when they may need the most support but paradoxically have least access to care.” At Cadence, managing these overnight alerts has proven effective. In a new analysis, Cadence’s Robert Ritchie shares data showing that for every 10,000 patients we support, our NP-led Care Team has over 11,000 monthly encounters, only ~40 of which result in an escalation to the ED or 911. Ultimately, Cadence helps patients unlock greater personal independence, confidence, and quality of life.     In a new piece in Healthcare IT News, Providence’s Chief of Virtual Care and Digital Health Eve Cunningham MD MBA and Family Practice Physician and Medical Director Dr. Kenneth Kooser discuss how RPM is improving patient health and reducing provider workload. Providence now has more than 1,500 patients currently enrolled in the care model with expectations for this number to reach 5,000 to 8,000 by the end of 2024. 88% of patients are taking their vitals daily and 77% remain enrolled in the program after six months. Dr. Cunningham shares of the partnership: "The program has since helped us keep chronically ill patients safe at home to achieve the Quintuple Aim of healthcare improvement: Improve the patient experience by tailoring treatments and interventions to patient data; address population health through proactive, preventative care delivery; lower cost by connecting patients to the most appropriate care intervention and reduce avoidable hospitalizations; improve provider well-being by extending their reach via a virtual care team; and provide equitable care by reaching all patients, regardless of their ability to present in-person for treatment."     With primary care shortages accelerating nationwide, Cadence’s integrated remote patient monitoring program is proving to be a key to better healthcare experiences for providers and patients. Dr Matthew Else, a family physician at Ardent Health’s Hillcrest Healthcare System based in Oklahoma shares his experience in implementing RPM to improve care for chronic disease patients between clinic visits: “By keeping close track of patients between their visits, I am better able to manage their conditions and improve their overall health more quickly.” Dr. Else adds, “The program is equally beneficial to me as a practicing physician. Knowing experienced clinicians are monitoring patients’ blood pressure, blood sugar and weight — and catching small trends in the wrong direction — has taken weight off my shoulders and greatly reduced the pressure on clinic staff.” Now monitoring more than 1,400 patients across Utica Park Clinic, we look forward to growing the program this year to extend this primary care infrastructure across the region. Read Dr. Else’s piece in HealthTech Magazines here: https://lnkd.in/e6ShAfQp

    This past Tuesday, Cadence’s partner, Providence’s Chief of Virtual Care and Digital Health Eve Cunningham MD MBA testified before the U.S. House of Representatives’ Energy & Commerce Committee in a hearing discussing patient access to #Telehealth services. In her testimony, Dr. Cunningham discussed the positive outcomes of Cadence’s program on Providence’s patients, sharing: “You can't effectively manage chronic condition diseases in an office visit that lasts 20 minutes every 2-3 months. It's this very frequent interaction where you can titrate, maintain and monitor your patients and tinker with their management to get them to good control. That is going to be the key to success in managing chronic disease." Dr. Cunningham added: “We’ve had so many great catches that we never would have caught if we hadn’t had this [remote patient monitoring] program deployed where the patients would have ended up hospitalized. We’ve seen a 3x increase in guideline directed medical therapy for our congestive heart failure patients, which is associated with a 70% reduction in mortality. The impact is significant. We have early signs for reduced EMS calls, reduced ED visits and readmissions in this patient population.” Cadence’s demonstrated results on chronic disease management and the total cost of care is one of many examples of Telehealth’s promising impact on the future of U.S. healthcare.     This past weekend, Cadence Medical Advisor Dr. David I. Feldman presented results showing the effects of Cadence’s diabetes program on blood glucose control at the American College of Cardiology’s #ACC24 conference. Results showed that 43% of type 2 diabetes patients on Cadence’s remote patient monitoring program achieved their blood glucose goal. “We’re excited about this early data showing the effect of a high-quality remote care program for patients with diabetes,” shared Feldman. “By combining patient-friendly, smart monitoring devices with an expert clinical team who provide proactive lifestyle and medication treatment recommendations, we are able to make meaningful improvements in diabetes care.     According to a new piece in Becker's Healthcare, Cadence’s partnership with Providence is “creating a paradigm shift in healthcare.” Chief of Virtual Care and Digital Health, Eve Cunningham MD MBA shares how Providence is leaning heavily into remote patient monitoring for its at-home care strategy. With over 1,500 patients currently enrolled in the care model, the program is expected to reach 5,000 to 8,000 by the end of 2024. Dr. Cunningham said the "secret sauce" of Providence's program is that it doesn't add to provider burnout. “The impact that we're able to make on getting patients into guideline-directed medical therapy, getting patients' hypertension under better control, getting patients' blood sugar under better control for diabetes — it's just amazing to see.
