I have a user seeing the default white page icon for all moz-icons, including icons for executables (instead of the default .exe icon). He's had the problem since Firefox 6, tried with a new profile and with a new install. This might be something to do with bug 415761 (user is Polish, I guess he's using cp1250 Windows?). I'll try to get him to join bugzilla and CC here.
