I’ve worked with a lot of different teams on a lot of different projects. And I’ve found that if a team or project is not succeeding - most of the time, it boils down to a people or process problem. To succeed, you need the right people in the right roles. Every project needs a: 1. Single Threaded Owner or STO - the person ultimately responsible for the project 2. Quarterback - who is responsible for daily project management 3. Working Team - the dedicated team 4. Leadership Team - leaders who give feedback on the work You then need to operationalize how all of these People work together with Process. This means: Setting up channels for the working team to communicate with each other and for the working team to communicate with the Leadership team. I agree that for a project to succeed everyone working needs to ground objectives , progress and obstacles in honest truth.

We've also found the MOCHA structure to be super helpful in clarifying people roles/responsibilities of our projects!
