Combined with disorientation in dense cloud cover near "terrain" (ground/mountain) and G-suit providing a surreal experience to the disoriented pilot, he pulled into the ground instead of the sky upon execution of "pull up". Modern days a HUD (pilot helmet) integrated with such tech provides real-world situational awareness not just of the environment, but also including friend and foe identifications.
