To my Health Center friends and colleagues….this picture is from 2015, when we fought tooth and nail for increased HC Funding to avoid going over the funding cliff…the very same scenario we face yet again, later this year.  At the time of this post, the House was EXTREMELY divided and there was near gridlock in Congress…sound familiar?! After this post, we went on to advocate in the Senate and won HC funding - averted the cliff, and secured access for an estimated 7 million patients whose care was on the line. But, here’s the most important thing…the power of advocacy was ever present and clear. The hard work, persistence and determination of health center advocates across the country produced an increase AND an extension in funding. SUCCESS was achieved through ADVOCACY, grit, commitment, passion and leadership. We can and will do this again in just a few months. Success is not just within our reach, It is the HC legacy - YOUR legacy! I’ve been honored to support and work along side health centers since 2005 - learning from countless colleagues who brought incredible vision and leadership, and who refused to give up - no matter how tired or unlikely success felt - they fought like hell because there were countless people still in need and their mission was to help. I was then, as I am now, inspired daily to keep doing this work because of their commitment. I post this to remind you of YOUR POWER, the power of your voice and your advocacy and the need to keep going! In 2016, ONE Charlotte Health Alliance (OCHA) was formed to bring Atrium Health, Novant Health and Mecklenburg County Public Health Department together to advance health equity across the Charlotte region. Over the past six years, OCHA has served as a convener to align partners and improve access to care in traditionally underserved areas of our community. Programming and operations associated with OCHA were supported through generous investments from those who understood the need, like The Duke Endowment, BlueCross BlueShield of North Carolina and other health care systems. This partnership created a foundation that proved particularly beneficial for our region’s coordinated response to community needs throughout the COVID-19 pandemic. As health care transitions into a new post-pandemic landscape, an independent convener is no longer needed to accelerate health equity work. While OCHA will no longer continue as an official organization, as of July 1, 2023, we are proud of the legacy that was started. Each health system and the partners involved are committed to transitioning programs and continuing to deliver quality services to our community as part of ongoing operations. THANK YOU to the individuals and organizations who have been a part of OCHA. The work you do in the community truly makes a difference and we know you will continue to be agents for change to improve quality and achieve equity in today's healthcare environment. 📢 Excited to share this insightful article on transforming eldercare and improving the lives of elderly individuals. 🌟 The U.S. healthcare system often fails to address the unique needs of economically disadvantaged seniors, resulting in loneliness, depression, and limited independence. However, there is hope in the Program of All-Inclusive Care for the Elderly (#PACE). This comprehensive, community-based model focuses on delivering personalized, home-centered care while reducing overall costs. One outstanding example is PACE Southeast Michigan (#PACESEMI), which has been recognized for its growth and quality metrics. With interdisciplinary teamwork and a focus on whole-person healing, PACE SEMI provides individualized services, fosters a sense of belonging, and reduces hospitalizations. The program's success lies in its culture of compassion and hiring staff who prioritize care over competence. Proactive investments in health are another key aspect of PACE programs. By addressing non-medical needs such as home modifications and community partnerships, participants' quality of life improves, and downstream costs are reduced. PACE SEMI's dedication to participants' well-being goes beyond traditional healthcare, exemplifying the transformative power of investing in people's lives. "Service with a Soul" is the guiding principle in PACE programs. By recognizing the worth of each individual and providing compassionate care, PACE fosters a sense of belonging and purpose. The program treats participants with dignity, allowing them to live at home with a higher quality of care and life. Expanding PACE offers a roadmap for transforming eldercare nationwide. By embracing comprehensive, home-centered care and investing in models like PACE, we can ensure that vulnerable older adults receive the support and compassion they deserve. In their new Forefront article, Robert Mechanic and Ashley Fitch from Brandeis University and The Mount Sinai Hospital discuss four key lessons learned from a group of ACO managers tasked with advancing their organizations’ efforts to integrate medical and social care. They advocate for additional federal investment in community care hubs and addressing the social determinants of health. "A recent study by Genevra F. Murray and colleagues examined 22 ACOs that were early adopters of initiatives to address the social determinants of heath (SDOH). The study, based on interviews conducted in 2015–18, concluded that despite strong interest, these organizations encountered significant difficulties integrating medical and social services. The principal challenges included: lack of data on their patient’s social needs and the capabilities of potential community partners; lack of mature partnerships between ACOs and community-based organizations (CBOs); and difficulty determining how to assess the return on investment of this work."
