Ms. Jones sat in her nephrologist's office, glancing over the Patient Activation Measure (PAM) survey handed to her. As she scanned the questions, she strongly disagreed with Question #4 and also showed reservations on Questions #10 & #13. Instead of merely collecting Ms. Jones' responses and moving on, her healthcare provider viewed this as an opportunity—a chance to truly understand and act on what the PAM was highlighting. The World Health Organization has long advocated for patient empowerment as a process where individuals gain significant control over health decisions. With the Kidney Care Choices model incorporating the PAM as a central quality metric, it's pivotal for nephrology practices to fully grasp its implications. This isn't just asking patients to complete a survey to satisfy a checkbox. It's about gleaning insights from these responses and translating them into actionable strategies for optimal care. Following the release of the KCC Model and inclusion of the PAM - nephrology experts responded that we need additional evidence that higher levels of patient activation are associated with clinically meaningful outcomes in kidney disease. The only way to determine the association is to actually act upon the survey results. In Ms. Jones' case, her reservations could lead to referrals to a pharmacist, thanks to her response to Question #4, and possibly a dietician for her answers to Questions #10 & #13. In essence, every PAM survey is an open door to improving patient activation and ensuring they receive comprehensive, tailored care.
