Memory Care Director:

Frontier Management is seeking an experienced Memory Care Director to manage the day-to-day operations of our Generations Neighborhood at Escalante at The Lakes community located in Las Vegas, NV. Demonstrated success as a leader in similar settings is required of the Memory Care Director.

The Memory Care Director insures the quality care of all residents in conjunction with the Executive Director and Resident Care Coordinator. The position requires an understanding and commitment to the principals and values of the community. This position requires creativity and flexibility with residents who have complex situations and is at times stressful. The Memory Care Director requires an ability and willingness to assist with the training of the Medication Techs in tasks required for a safe and complaint medication delivery system. The Memory Care Director position requires a caring nature and willingness to care for the elderly.

DUTIES AND RESPONSIBILITIES:

  • Assure smoking and safety evaluations are completed in conjunction with quarterly assessment.
  • Implement interim service plans as care needs change and assure all medication techs are in-serviced on implementing interim service plans as needed.
  • Assure home health, hospice and ancillary care providers, communicate their visits and provide necessary documentation of their visit.
  • Complete MAR audits.
  • Other department audits as directed by Executive Director.
  • Assist with tracking and recording incident reports. Assure the incident investigations are completed.
  • General record keeping and filing as needed.
  • In conjunction with the Executive Director, hire, train, supervise, counsel as needed, and terminate the care staff as conditions arise.
  • Work as manager of the day as directed by Executive Director.
  • In conjunction with Executive Director, perform evaluations, counsel, discipline and discharge if necessary, all health care staff.
  • Perform such other tasks as may be required from time to time by the management of the property.
  • Complete delegation training, assessments and documentation per state guidelines.


DAILY:

  • Read the 24 hour communication book and respond as necessary to issues.
  • Review all incident reports and residents on alert status.
  • Read any new interim service plans that could have been initiated.
  • Meet with RCC to review care changes, staffing, and resident needs.
  • Review alert charting log.
  • Ensure service plan meetings are completed timely and per state regulations. Attend meetings if resident has had a change of condition or change of care level.
  • Assist with orienting all new caregiver/medication tech staff along with RCC/ED.
  • Answer to resident/family needs as issues arise.
  • Attend daily stand up meetings, ensuring that the needs of the residents have been met.
  • Assess residents that have experienced a change of condition. Ensure appropriate documentation and service planning have been completed.
  • Ensure that all Health Services staff have appropriate training.


WEEKLY:

  • Audits as per schedule, (MAR/TAR)
  • Add interim service plan care needs to main service plan as care needs change.
  • Monitor and maintain wound book. Assess, document and service plan as needed.
  • Complete LPN tasks that cannot be delegated to staff.


MONTHLY:

  • Attend and participate in the scheduled mandatory in-services
  • Attend and participate in other scheduled meetings (Safety, Quality Assurance)
  • Audit med room procedure and practices, including one medication tech (for compliance and infection control practices related to medication administration.
  • Train staff in regards to handling emergencies.
  • In conjunction with Executive Director and the Resident Care Coordinator, ensure health services department is compliant with state regulations.
  • Attend and assist with general orientation of new hires.


QUARTERLY:

  • Complete quarterly resident assessments.
  • Complete Psychotropic medication reviews.
  • Review Resident medication regimen.
  • Other LPN assessments as needed.


Universal Precautions:

  • Exposure to blood/body fluid is likely.


PHYSICAL REQUIREMENTS:

  • Must be able to easily lift and carry 50 lbs. Must be able to stand, walk, stoop and/or bend for periods of up to eight hours with breaks as provided for in the employee handbook.


OTHER REQUIREMENTS:

  • Must be a Licensed Nurse in the appropriate state.
  • Must possess a current state specific driver's license and appropriate driving record.
  • Proficiency in English preferred. Attends in-services monthly required for position. Receives and maintains CPR certification.
