Director of Social Services responsibilities Include:

As the Director of Social Services some responsibilities include:

• Orients and advocates on behalf of the residents and their families to the nursing center while offering continued support.
• Completes social history and MDS assessments, and develops care plan, which identifies medically related social and emotional problems and needs with realistic goals and specific actions to be taken
• Facilitates discharge planning, from time of admission, for short-term rehab patients.
  
  
  

Qualifications:

• BS or similar in social work
• Serious applicants
• Effective Communicator
• Previous LTC/SNF experience preferred
• Compassionate working with the geriatric population
