About the Role:

The Director of Care Coordination will be responsible for the overall strategy and success of the Care Coordination team, directing daily operations and the cross-functional development of related products. As Director of Care Coordination, this role will guide the organization on best practices and ensure tools, processes, and standards are aligned with operational and industry excellence, and continue to evolve the team to deliver against customers' contracts and industry needs. This role is responsible for leading and managing a team of managers who support the care coordination and facilitate seamless connections between clients seeking services and our network of partners

The Director will work closely with Care Coordination leadership and cross-departmental leadership to develop and implement strategic processes to advance the scope and functionality of the Care Coordination department as part of Unite Us’ end-to-end solution. The Director will ensure all team operations support the larger customer and business needs, establish care coordination metrics for performance, and develop clinical-based workflows including screenings, assessments, reporting, and managing all systems and tools.

What You'll Do:

  • Lead the Care Coordination team, ensuring the strategic and operational activities of the team are designed and developed in line with business objectives, customer and industry standards
  • Maintain and evolve the Care Coordination offering by collaborating with Product, Customer Success, Sales, and Business Analytics to enhance and build out processes, product offerings, and support customer deliverables and escalations
  • Strategically collaborate with customers to develop policies, procedures, and workflows, managing relationships and change effectively
  • Develop and maintain standard operating procedures to ensure clinical adherence, compliance, and efficiency across the care coordination team
  • Manage tools, technology, and contracts supporting care coordination
  • Cultivate a positive work environment fostering team empowerment, goal achievement, and company success
  • Maintain team structures and meet metrics to consistently fulfill contractual requirements
  • Cultivate strong relationships with Care Coordination leadership, Customer Success, and cross-departmental leaders, serving as a Care Coordination strategy expert
  • Escalate and manage operational and strategic contractual risks to senior leadership, enabling clear escalation pathways to be followed
  • Address and manage issues with internal tools
  • Demonstrate proficiency in Unite Us' end-to-end solution
  • Drive continuous improvement initiatives to enhance team efficiency and effectiveness
  • Coach and develop Care Coordinators at all levels, promoting collaboration and professional growth
  • Champion a customer-centric culture and deliver excellence in customer experience by meeting and exceeding service expectations and solutions workflows that drive impact
  • Support the executions of backend workflows and special projects as needed
  • Collaborate with the product in testing and providing user feedback

You’re a great fit for this role if:

  • Minimum of 5 years of leadership experience managing teams and guiding projects.
  • Clinical license such as LCSW, RN, LMHC, Certified Case Manager
  • Experience in leading and growing a care coordination team in public health, healthcare environment, health insurance payor environment, or related industry with regards to the coordination of care.
  • Proficiency in navigating social services, managing care coordination/care management workflows and teams
  • Experience in navigating social services as an individual, case manager, social worker, or care navigator
  • Ability to implement best practices and influence organizational scaling
  • Demonstrated ability to understand business drivers and implement supporting strategies
  • Expert ability to synthesize complex data and analysis into clear, concise, executive-level presentations that frame decision options concisely.
  • Proven ability to work collaboratively across departments
  • Critical, strategic, and operational thinking skills
  • Excellent organizational management skills with the ability to build relationships, develop high-performing teams, and set and achieve strategic objectives
  • Empathy and passion for helping vulnerable populations
  • Advanced computer skills and relationship management abilities
  • Ability to prioritize and work independently

Environmental Job Requirements and Working Conditions:

  • This position is remote, U.S.-based
  • The target pay range for this role is: $120,000-$140,000. This salary range represents our target hiring range for this role. The proposed salary will be dependent on the candidate's skills, experience, and competencies, as well as location.
  • This position may require 10% travel
  • All team members will be required to pass a background check which includes criminal, employment, and education verification
