There are many points to safety that make it complex, but not impossible, to achieve. The Centers for Medicare & Medicaid Services (CMS) proposes to adopt the Patient Safety Structural Measure (PSSM), an attestation-based measure that assesses whether hospitals demonstrate having a structure and culture that prioritizes patient safety.
