Senior Social Worker £40,221 to £43,421 pa As a Senior Social Worker you thrive on close collaboration with partners and taking a holistic approach to individuals’ needs, putting support in place so they can be cared for at home rather than in hospital. We have two vacancies in the hospital discharge neighbourhood teams, which take a person-centred approach to assessments, interventions and support plans. We work in close partnership with NHS and third sector colleagues, and we are adopting the Home First model to enable more patients to be at home instead of in a healthcare setting, receiving the care they need to promote their recovery and stay safe. As well as managing your own caseload as a Senior Social Worker, you will guide and mentor less experienced team members and contribute to planning service developments and innovations. You will hold a professional social work qualification and be registered with the Health and Care Professions Council on commencement in post.
