So, how has Spine_Endo changed our decision making, cost containment, and exactly who are the winners and loosers here? Case: 67F physician. Gait and sphincter dysfunction from progressive T10/11 cord compression….. 5 years ago, I would have made a midline cut, “standard” decompression, then screws / graft for fusion. Now? Simple = unilateral uniportal decompression. No hardware. D/C 11 hours post op. Winners & Loosers?? Winner 🥇 = The Patient. Looser #1 = The Surgeon = I get paid less :( Looser #2 = The Hospital = get paid less :( Looser #3 = The Company = no hardware :( Don’t get me wrong, I still love my hardware, but endo has changed my practice profoundly. My fusion rates have dropped considerably. On the topic of costs, I keep hearing that “endo costs more”. What total and utter rubbish!! Let me be plain and simple = less fusion + less hospital stay + rapid return to full activity + less complication profile + less follow up costs + less PT = less cost!! I challenege anyone out there to convince me otherwise. I am collecting data to confirm this, but may take me another 18+ months to get there….
