Description: The Case Manager, as part of the patient care team, provides direct case management of all assigned patients; thereby contributing to the improvement of patient care, their quality of life, and treatment compliance by the patient. The Case Manager serves as a liaison and advocate for the patient and family, ensuring the delivery of quality and timely services to the patient from the patient care team and/or supporting government and community agencies. El Rio Health APRN Residency Program has openings! This is a 12-month program, full-time employment opportunity; with a 2-year post-residency employment commitment at El Rio or one of our affiliated FQHCs. This program is designed for Family Nurse Practitioners (FNPs) who have graduated within 18 months to the start of the Program (September 9th, 2024). You do not need to be licensed to apply, most of our graduates are new graduates, but must be licensed by July. As the largest FQHC in Arizona and the 19th largest in the nation, we are leaders in the integrated healthcare community. HRSA funding has allowed us to collaborate with two smaller FQHCs to create a pipeline of highly skilled FNPs who are equipped to serve individuals in a rural setting near the United States/Mexico border to enhance the Southern Arizona primary care workforce.
