“Prolonged stays in hospital are bad for patients, especially for those who are frail or elderly. Spending a long time in hospital can lead to an increased risk of falling, sleep deprivation, catching infections and sometimes mental and physical deconditioning. Despite this, nearly 350,000 patients spend more than three weeks in acute hospitals each year.” NHS England - Reducing length of stay (https://lnkd.in/ebctmDaE) Powered by the Voror Health Platform, the NHS 111 Discovery frailty flagging project utilises Discovery data to flag key information to NHS 111 call handlers to improve the patient pathway and experience, and reduce the unnecessary use of emergency services and admission to A&E. Since the project went live in November 2018, the service has: * matched 4,219,720 patients * flagged 286,429 potentially frail patients * processed 24,593,824 API calls That's 286,429 potentially frail patients that have avoided a potentially unnecessary trip to A&E or a longer stay in hospital.

It was a great privilege to speak on behalf of suppliers at the recent 2nd National Shared Care Record Summit at the International Convention Centre in Birmingham, hosted by Yorks & Humber Care Record. I did that, giving raw feedback and went a bit further!
