Derm Foundation Access Form

Thank you for your interest in Derm Foundation. This tool generates custom embeddings from dermatological images. These embeddings can be used to develop custom machine learning models for dermatology with less data and compute compared to traditional model development methods.

Please complete the form below to apply for access to the Derm Foundation. You can apply as a group or as an individual; please refer to the README for instructions. If you are approved to use the Derm Foundation, your use is subject to the Google Terms of Service and Derm Foundation - Additional Terms of Service that appear below. By completing this form, you consent to collection of your personal information submitted, which will be used to evaluate and select researchers who meet the participation criteria, as well as for record-keeping purposes.  The Derm Foundation Team will have access to the information that you submit, including your name, email address, and institutional or organizational affiliation.  Your submission data will be subject to Google’s Terms of Service and Privacy Policy. Your personal information will be maintained indefinitely for auditing purposes, and to ensure compliance with the Terms (as defined in the Derm Foundation - Additional Terms of Service) and other regulatory requirements.

If you meet the participation criteria, you will be contacted via email and provided with additional details and next steps. Please see the README for the requirements on options for setting up access through an identity. If you'd like to run the tool on your own data you must have a Google Cloud Platform (“GCP”) account and store that data in a Google Cloud DICOM store. This is not required to run the demo Colab. Completing this form does not mean that you have been selected to use the Tool.

To use Derm Foundation, you must accept (1) the Google Terms of Service, and (2) these Derm Foundation - Additional Terms of Service (the “Additional Terms”). Please read each of these documents carefully. Introduction

Derm Foundation generates custom embeddings from dermatological images. These embeddings can be used to develop custom machine learning models for dermatology with less data and compute than other approaches.   

Use Restrictions

Do not use Derm Foundation, or any embeddings you generate from Derm Foundation, to provide patient care.  For example, you may not use Derm Foundation, or any embeddings you generate from Derm Foundation, as or in a medical device.     

Do not use Derm Foundation if your use of Derm Foundation, including your sharing of content with Derm Foundation, would conflict with any commitment you’ve made to a third party. 

Do not use Derm Foundation, or any embeddings you generate from Derm Foundation, to: 

  • recreate any training data used to develop the underlying model;

  • decompile, unlock, disassemble, or otherwise translate the software or any underlying models; or

  • generate any identifying information about an individual.

Sharing Content with the Tool

While you have no obligation to upload or share any content with Derm Foundation, if you choose to do so, please make sure the content doesn’t contain any identifying information - such as Protected Health Information under HIPAA or Personal Data under GDPR.  For example, do not upload or share any dermatological images to Derm Foundation that include a patient medical record number. 

This requirement is in addition to the requirements under the Google Terms of Service that you have the necessary rights to any content you upload or share with our services and that the content is lawful.

Suspension or Termination

In addition to the situations where we reserve the right to suspend or terminate your Google Account under the Google Terms of Service, we may suspend or terminate your access to the Tool if we reasonably suspect your use may cause harm to a third party, such as a patient. 

Cloud Platform Services

Your use of Google Cloud Platform in connection with Derm Foundation is subject to the Google Cloud Platform terms you entered into with Google. 

Acknowledgments

If you make any publication related to your use of Derm Foundation, please cite Google's Derm Foundations tool. However, you may not use Google’s name to endorse or promote your work without our permission.  To request our permission, contact us at [email protected].

Definitions

“GDPR” means (i) the European Union General Data Protection Regulation (EU) 2016/679 (the “EU GDPR”) on data protection and privacy for all individuals within the European Union (“EU”) and the European Economic Area (“EEA”); (ii) the EU GDPR as incorporated into United Kingdom (“UK”) law by the Data Protection Act 2018 and amended by the Data Protection, Privacy and Electronic Communications (Amendments etc) (EU Exit) Regulations 2019 (“UK GDPR”); and (iii) the Federal Data Protection Act of 19 June 1992 (Switzerland) (each as amended, superseded, or replaced).

“HIPAA” means the Health Insurance Portability and Accountability Act of 1996.

“Personal Data” has the meaning under the GDPR.  

“Protected Health Information” has the meaning under HIPAA.
